Licensed Practical Nurse (LPN)- North Shore

POSITION: Licensed Practical Nurse (LPN)

SCHEDULE: M-F 9-5; on-call Mon 9am through Fri 5pm and rotating on-call

SALARY:  $30-$33 hourly +sign on bonus

LOCATION:
North Shore

RESPONSIBILITIES:
•    Establishes, maintains, and updates all individual’s medical records
•    Writes comprehensive evaluations, completes all consult forms, writes progress notes and maintains health care record for the             individual's ISP     
•    Obtains medical reports and approval of treatment plans, written orders for medications, treatments, etc. from the individual’s               physicians    
•    Provides on-going training and in-service education to management and staff regarding general medical, psychiatric, neurological       and nutritional issues. Trains staff in vital signs, blood glucose monitoring, and other medical treatment as needed
•    Maintains consistent communication with the clinical consultant assigned to the program, as well as a communication system with       the families and guardians of individuals, their day program staff and the DDS Service Coordinator 
•    Completes pre-admission review of medical history and needs of prospective individuals to reside in Turning Point, Inc. programs
•    Maintains an adequate medication supply in the residential program at all times
•    Ensures adherence to all DDS and MAP regulations regarding medication administration, documentation, and storage  

 QUALIFICATIONS:
•    Current licensure as a licensed practical nurse in the Commonwealth of Massachusetts
•    At least (2) two or more years’ medical/surgical and/or psychiatric nursing background
•    Experience caring for developmentally disabled individuals is strongly preferred
•    A solid knowledge of nursing principles and procedures, as well as good patient care techniques 
•    The ability to attend individual’s medical appointments and related meetings is necessary
•    Must be able to be on call on a 24 hours/day and (7) day/week basis in order to respond to medical/health questions or concerns
